Initial enquiries
The carer intake form collects the details and documents you choose to submit for recruitment review. Complete the health, new employee and confidentiality stages only when Chommzy asks you to. A form submission is not an offer of employment.
Appropriate stages
Only provide information relevant to the current recruitment stage through the route agreed with the team. Detailed health questions and role-related checks must be handled at an appropriate stage. A request for interview adjustments can be discussed separately.
No automated selection
This website does not score applicants, make employment decisions or evaluate protected characteristics. It has no automated applicant scoring or selection system. Optional monitoring information is for authorised HR review, and any future-opportunity retention follows the choice you provide.

As you type, answers are saved in this browser for up to 24 hours since the last edit. This can include the personal, employment and health details you enter.
